SpeakUP Consent Form

Lifestart welcomes the opportunity to work with you and your child on the SpeakUP program.

This program will be delivered in person at school, and facilitated by a Lifestart staff member who will be assisted at all times by a school representative such as a school teacher or a student support officer. In the situation where a school staff member cannot attend the session, the session will not go ahead. An opportunity to reschedule will be given if possible. There may be unforeseen situations where sessions will be conducted through online mediums such as Zoom or Microsoft Teams.

Lifestart will not use any images or videos of your child or you for any other purposes, including for marketing and promotional purposes, without gaining your separate, informed consent.

On the rare occassion that a session may be videoed parent consent will be requested prior and is not considered as part of this consent. Lifestart will always keep your information confidential. The only reason we will ever share information without your permission is if we are required to do so by law.

Please complete this form at least a week before the beginning of your child’s first SpeakUP session.
