If you have a child under 9 without an NDIS plan and you’re looking to access the NDIS Early Childhood Approach, please do not use this form. You can lodge a referral for NDIS support through the Lodge your referal form.

If you do have an NDIS plan, or are looking to pay for support with private health cover, medicare rebates under a CDMP, or pay the full service fee yourself, fill out the form below.
