Transitioning to High School – short term supports

Lifestart is offering dedicated short term supports to young people who will be moving into Year 7 in 2024. 

These individual supports will be goal focused on the transition and, settling into the new high school, and will be directed by the young person with their goals.  

We will be offering up to 6 sessions online with a Lifestart therapist over the January holiday break and into February and March 2024. These sessions can be paid for using NDIS funding or fee for service. Your young person may like to have all their sessions before going to high school or might like to space them out over the period up until the end of March 2024. 

The benefit of receiving supports in this way is that they are personalised to the young person as the goal is theirs and that the supports are received in a timely way to support a transition that is sometimes tricky to navigate alone. Our therapists will use a coaching and strength based approach and will be upskilling the young person and parent/carer to be their own advocate. We can also look at the supports that are available in a high school and work through with the young person their best contacts at the school for getting support and assistance when they need it. 

If your young person would like to be considered for these supports starting from 10 January 2024, please contact the Client Engagement team on 1800 953 390 and place an expression of interest with the team. You will then be contacted before we break for the Christmas close down period and allocated a transition facilitator to work with you and your young person.
