Lifestart and KEYS partnership

KEYS Network Navigators are invited to use this web referral form to refer the children and families that they support to Lifestart for short-term therapy supports. Please ensure that you have family consent and approval for brokerage funding from your Team Leader (KEYS Network). Please provide as much information as possible. The information will be forwarded to our Lifestart team and we will be in touch with the family within the next 1-2 weeks to arrange an initial appointment. Appointments will generally be on Fridays at the Mt Druitt Hub or Parramatta Hub.
